I am no expert in this area but as I understand it, prior to the ACA, companies would offer "dirt-cheap" policies for healthy persons that covered virtually nothing with high deductibles.

The ACA raised the bar and required policies cover preventive care, etc. in order to ensure that individuals actually had coverage for the kinds of things they would likely need to use.

It is a double-edged sword. You allow policies with virtually no coverage for cheap or you require some basement level of coverage and prices rise.
