For all the reasons you stated and more! there are medical conditions treated with contraceptives that also cost way more to treat if contraceptives can't be used. For example, if Endometriosis isn't treated with oral contraceptives the insurance companies will eventually be paying for laparoscopic surgery, infertility treatment, etc.

I think insurance companies would be well within their rights to charge more for any policy that doesn't include contraceptive coverage.
