12. I don't understand this joke at all.

Somebody please explain it to me. And please explain it to me as if I'm a "dim bulb" because apparently I am. I have absolutely no idea at all what this joke is about. I don't get the setup, I don't get the punch line, I don't get any of the subsequent comments. And what does GMTA (Floyd R. Turbo's comment) mean?
